Here’s all you do once you’ve completed your sleep test:

Remove the battery from the WatchPAT® ONE device and put the device back into the box
Fill out the below form. A pre-paid return label will be generated
Print the pre-paid return label and attached it to the box
Ship it back to us – completely free of charge!
